Posted because it is from an official source.
Not speculation.
The document is a safety notice to all operators of similar vehicles to do an urgent check on the brake gear mentioned.


Similar notices are issued by NWR and other bodies on a variety of subjects to all interested parties.
Often in advance of any official notice from RAIB or similar.

The notice could have added that the use of wheel scotches would be a sensible precaution.
