I have also known this to happen pre Railtrack under BR.

The steam crane that resides currently at Embsay is an example of the result.
The Crane had been working on a PW job between Malton and Seamer. Work completed all Ok.
Possession was given up and all PW trains departed.
However the Engineering Supervisor had forgotten that he had detached the crane while tipping stone.

The PICOP has to take on trust any statements from the ES that his worksite is clear, as he may be miles away even in those far off days, well 1980s.
No Track Circuits at the location and the result was the first train of the day found the crane the hard way.

The ES kept his job somehow, but he never got any further.
