Mystery mishap

I'm trying to identify this loco and the possible scenario, as it has some relevance to my gggrandfather, though I don't yet know what!
It is almost certainly an NER engine that has been derailed and the number on the cab is 474. Anyone offer a clue? The scene is possibly in the Hartlepool area, sometime between 1900-1920 I guess. Any help appreciated.

474 NER class T built April 1903, renumbered 3278 in March 1946 then became 63278 class Q5 under BR. Withdrawn August 1950.
